- Middle School Camp Online Registration Form – for current 7-8th graders
- High School Camp Online Registration Form – for current 9-10th graders
• NOTE: Your complete online registration & payment are required to secure your space (first-come basis).
• Registration deadline is April 30.

• If your child qualifies for free or reduced lunch but lacks documentation, complete this income qualification form. Otherwise, just send the school letter.

- Waiver of Liability – Required by all
- Behavior & Safety Agreement – Required by all
- Emergency Medical Authorization – Required by all
- HS Camp Field Trip Permission Slip – Required for all High School Campers only
- Non-Prescription Medication Form – Required for over-the-counter meds brought to camp
- Prescription Medication Form – Must be signed by your doctor; one per medication
Note: Medications are administered at breakfast, lunch, dinner, or nighttime snack. Provide the camp nurse with medications in the original packaging and just the dosage to be used at camp.
Completing Forms Digitally
Most forms can be completed and signed digitally — except the Prescription Medication Form (requires a doctor’s signature) and the Emergency Medical Authorization (requires photo attachments). Email completed forms all together, noting the campers grade-level (MS or HS program).
