Financial Aid Scholarship Application
(If your child qualifies for free or reduced lunches at school, but you have no qualification letter – then complete this income form to demonstrate financial need. Otherwise just send the school letter.)

Non-Prescription Medication
(Required for over-the-counter medicines brought to camp; not required for incidental OTC medications approved on the camper’s registration form & provided by the camp nurse, i.e. Tylenol, Advil, Benadryl, Safetec Sting Relief, hydrocortisone cream, Tums, Delsym, Kaopectate)
Prescription Medication
(Required if the camper will have prescription medication at camp; must be signed by your doctor.)
NOTE: Medications are administered at breakfast, lunch, dinner, or nighttime snack.
Some of the documents can be completed digitally — except the Prescription Medication form must be completed and signed by your doctor and the Emergency Medical Authorization requires photos/images. Forms have fillable fields, so you can enter your information and sign documents on your device. It will probably work best on a computer, and you might need to download the file in order to edit it. Or you can also print documents; either email a photo or scan of the completed document or mail the paper form.
