APPLICATION INFORMATION:
B-WISER Camp – Online Registration
     • Complete online registration & payment are required to secure your space at camp on a first-come basis.
Financial Aid Scholarship Application
     • If your child qualifies for free or reduced lunches at school, but you have no qualification letter – then complete this income form to demonstrate financial need. Otherwise just send the school letter.

Waiver of Liability
     • Required by all
Behavior & Safety Agreement
    • Required by all
Emergency Medical Authorization Form
    • Required by all
B-WISEST Field Trip Permission Slip 
    • Required by all B-WISEST (rising 10-11th grade) campers; not needed for B-WISER
Non-Prescription Medication Form
    • Required for over-the-counter medicines brought to camp; not required for incidental OTC medications approved on the camper’s registration form & provided by the camp nurse, i.e. Tylenol, Advil, Benadryl, Safetec Sting Relief, hydrocortisone cream, Tums, Delsym, Kaopectate.
Prescription Medication Form
    • Required if the camper will have prescription medication at camp; must be signed by your doctor.  A separate form is needed for each medication.
NOTE: Medications are administered at breakfast, lunch, dinner, or nighttime snack.

Some of the documents can be completed digitally — except the Prescription Medication form must be completed and signed by your doctor and the Emergency Medical Authorization requires photos/images.  Forms have fillable fields, so you can enter your information and sign documents on your device.  It will probably work best on a computer, and you will need to download the file in order to edit it.  If you are having trouble entering information on the electronic form, please click here for some suggestions.  Or you can also print documents; either email a clear photo or scan of the completed document or mail the paper form.
